Indiana Finds Nearly $200 Million in Improper Payments in Attendant Care Audits

(INDIANAPOLIS) - State officials are seeking to recover nearly $200 million in improper Medicaid payments tied to home-based care providers.

According to the Indiana Capital Chronicle, the Indiana Family and Social Services Administration is pursuing repayment from five of the state's largest attendant care providers after audits found widespread issues.

The review covered claims submitted between January 2022 and March 2025 and was launched after a $150 million spike in spending that officials say could not be explained by changes in patient need.

Auditors examined 625 claims using a statistically valid sampling method and found errors in nearly all of them.

Issues cited include billing for services not allowed under the program, missing or incomplete documentation, and problems with authorizations.

Officials also raised concerns about incomplete or missing background checks for caregivers providing in-home services.

FSSA has sent letters to providers seeking repayment, though appeals are expected.

The agency says it plans to expand audits, increase oversight, and implement additional review measures to prevent future improper payments.
